We will hold a selection for current 6th graders based on the following details to form the 2023 U-15 Musashi team.

□Eligibility
Current 6th graders (born between April 2, 2010 and April 1, 2011), both field players and goalkeepers
※We will generally ensure that you can commute to the practice location (Tokyo Gakugei University Attached Koganei Junior High School) within 90 minutes from your home.

・Please log in with your J.League ID and enter the required information.
※If you do not have a J.League ID, please obtain a J.League ID on the application form authentication page and then apply.
※Only those who have confirmed and agreed to the "Guidelines for Preventing the Spread of the Novel Coronavirus" in the application form can participate in the selection.
※After completing your registration on the application form, the application completion screen will be displayed.
※After the application period ends, we will use the email address, postal code, address, and phone number registered with the J.League ID as the contact information for this selection. If there is no registration of the necessary items or if there are any changes in the content, please make the necessary modifications before the application period ends.
※Please note that if you remove FC Tokyo from your favorite club settings before the application period ends, we will no longer be able to send you information from FC Tokyo.
※Please set your email settings to receive emails from @fctokyo.co.jp.

③After the payment process is completed, you will receive an email titled "Confirmation of Ticket Application Details" from J.League Ticket (jleague-ticket-auto@pia.co.jp).
※ Please set your settings to receive emails from "jleague-ticket-auto@pia.co.jp".
※If you do not receive an email, please log in to the J.League Ticket official website and check if the payment process is completed from the "Purchase History" in the upper right menu button.

④ Purchase a "Selection Participation Ticket" at Seven-Eleven stores.
※Please provide the "payment slip number (13 digits)" to the store register when issuing tickets.
Please inform the staff that you are making an "Internet purchase" to proceed smoothly.
※A system usage fee of 110 yen and a ticket issuing fee of 220 yen will be charged per ticket, and there may be additional payment fees depending on the payment method.
※Please note that if you choose to pay by credit card (including d payment, Rakuten Pay, and Yahoo! Wallet), you will not be able to issue tickets at Seven-Eleven until 24 hours after the payment for the prevention of fraudulent use.
※If you want to issue a ticket within 24 hours due to the deadline, please choose "Payment at Seven-Eleven".
◇ If you have forgotten your "Payment Slip Number (13 digits)"
Please log in to the J.League Ticket official website and check from the "Purchase History" button on the top right menu.

⑤Download the "Examination Acceptance Form" from the following and make sure to obtain the signature and approval stamp from the representative of your affiliated team (such as the coach).
※Currently, those who are not affiliated with a team do not need the representative's signature or consent stamp.
※Please download the PDF at the bottom of this page for the examination consent form.

⑥ Paste the "Selection Participation Ticket" issued at Seven-Eleven on the "Examination Acceptance Form" and send it to the address below.

□Reception Confirmation
Those who have completed both the 【Registration of the Application Form】 and the 【Mailing of the Examination Acceptance Form (with the participation ticket attached)】 will be sent examination instructions by July 14 (Thu) to the email address registered with their J.League ID as of the application deadline (Wed, July 6).
※Please note that if you remove FC Tokyo from your favorite club settings before the application period ends, we will no longer be able to send you information from FC Tokyo.

※If you do not receive the notification email even on Jul 15 (Fri), please check your spam folder and contact the selection department below.
